I have been a Registered Nurse since 1991 and thus have 19 years of working experience and my specialty area is Anaesthetics and Recovery Room (PACU). This field has given me the chance to work with various members of the multidisciplinary health team and varied clientele in different specialties. I am therefore very comfortable working as Anaesthetic Nurse, Recovery Room Nurse or an Anaesthesia Technician.

I have learnt to assess, plan, implement and evaluate nursing care delivered. My several years of experience within established organization have made me well used to working to high standards in a pressurized environment. As a committed and conscientious employee, I am keen to develop my career and levels of responsibility.

Day before surgery:
• Pre-op visit to establish rapport with patient and relatives, assess patient system by system, checking of vital signs, check results of laboratory investigations done, then make a choice of anaesthesia with patient or relative if patient is a minor.
• Prescribe pre medications, as may be necessary then grading according to American Society of Anaesthesiologists (ASA) grading. Consult surgeon for discussion where necessary
Day of surgery:
Visit patient on the ward then check vital signs. Check Anaesthetic machine and equipment needed depending on the choice of Anaesthesia. Get a functioning suction machine tested and made ready.
• In theatre, receive patient from ward nurse then check for consent by patient or relative if patient is a minor.
• On theatre table, attach monitors, check baseline vital signs (Blood Pressure & Pulse) and record, venepuncture (IV line) for drugs, pre oxygenation, induction of anaesthesia, endotracheal intubation or use of laryngeal mask airway for general anaesthesia, lumber puncture if spinal anaesthesia after preloading, nerve block, local infiltration, total intravenous anaesthesia (TIVA), Monitoring and recording of patients vital signs, monitoring the flow of i. v. fluids, oxygen and inhalation anaesthetic agent into patient, observe urine output, observe the operation site for oxygenation estimation of blood loss and progress of surgery. Keeping of anaesthetic record chart, blood transfusion when necessary, writing of post anaesthetic instructions.
• Handing over patient to recovery room nurse and giving of verbal instructions to stress important points if necessary.
-To maintain a patent airway by chin lift or jaw thrust if not contra indicated.
- Prevent aspiration by suctioning when need be.
- Prevent physical injury by keeping side rails up and padded for paediatrics.
- Provide comfort and warmth
- Manage pain
- Monitor and record vital signs accordingly.
- Maintain good circulation.
• Post operative visits on the ward.
• Requesting and collecting drugs from pharmacy and ordering for items
from the hospital medical store.
• Care of anaesthetic machine and equipment. Checking for expiry dates of drugs and replacing such expired drugs at the end of every month.
• Assist on the ward to give resuscitation to new born babies and during emergencies, venepuncture, cutdown, oxygen administration, lumber puncture, minor suturing, incision and drainage, removal of foreign bodies in the nose and ears etc.

Check and record functioning of glucometre each morning before the start of work, check availability of oxygen both pipeline and cylinders, check efficiency of monitors, check the availability of items and drugs and restock. Help to check and issue out controlled drugs for the day's use, make ready a functioning suction machine and ambu bags of different sizes. Check and replace expired drugs at the end of every month.

Ensuring that PACU and its environment is quiet with minimal lighting. Receive patient from operating room after surgery, ensuring a patent airway and breathing by lifting the chin or jaw thrust, maintain circulation. Attach monitors for total management of immediate post-op patients recovering after spinal, local/regional/ and general anesthetics. Check to see if an airway is insitu and the type of airway (oral, nasal, LMA, ETT), check for any allergies. Put in the appropriate and comfortable position as ordered depending on the anaesthesia given and surgery done. Always stay with patient and ask for assistance incase patient becomes restless.

Frequent assessments and recording at 5 minutes interval. Invasive and non-invasive monitoring of blood pressure, pulse, respiration and temperature, Pulse oxymitry for oxygen saturation. Assess PAR, sedation and pain scores and record then manage pain as ordered by either setting up a PCA pump, pain buster or epidural pump. Provide warmth and protect from injury by keeping the side rails up and padded in paediatric surgeries.
Check if drain is insitu and type, check dressing if soaked, checking of eye shield if in place, check cast, sling and tightness of bandage, check peri-pad if insitu, check intake and output, measure amount of urine and blood and drainage and estimate mount of blood loss on dressing and peri-pad and record. Check iv line, check dermatone level in spinal analgesia. Manage nausea and vomiting as ordered and document outcome.

Assist with intubation, extubation and reintubation, lumber puncture, epidural, setting up a central and arterial lines.

Implement all surgeon and anaesthetist orders and record accordingly with all findings and patient's condition in PACU, Sign and ward nurse signs before handing over to ward nurse when patient is ready to be sent to the ward.

• Daily ward cleaning, daily bed making, ward rounds with medical officer, admission procedures, explaining every procedure to the patient and relatives, checking and recording of vital signs (temperature, pulse respiration and blood pressure).
• Wound dressing starting with the sterile or neatest first using aseptic technique, cleaning and sterilization of used instruments, bed bath, checking and treating of pressure areas/ulcers, care of the head, hair, face, neck and nails. Serving of bed pans, urinals and sputum mugs.
• Serving of medication (oral, intramuscular, intravenous, rectal, topical etc), vein puncture, checking of expiry dates of drugs, serving of meals, passing of urethral catheters, passing of naso gastric tubes, tube feeding, checking and recording of intake and output. Nursing patient in the appropriate position depending on the patient’s condition.
• Setting of trays for lumber puncture, PICC line and blood transfusion. Oxygen administration and resuscitation, preparing and sending patient for X’ray, collection of specimen, labeling of specimen, filling of laboratory forms and sending the specimen to the laboratory for investigation, going back for the laboratory results when ready. Going to the pharmacy for drugs and going to the medical stores for supplies.
• Preparing (explain procedure to patient, signing of consent, removal of jewelry, dentures, checking of vital signs, giving pre-medication if prescribed etc) and send patient to theatre for surgery, handing over patient to theatre staff, going back for the patient from the recovery room after surgery and recovery from anaesthesia.
• Education of patient on home medication, life at home after discharge, coming back for review, how to get in touch with the hospital when in need etc.
• Discharge procedures and LAST OFFICES.
