Nature of Duties

Duties and Responsibilities - Permit Controller:
Establish a SIMOPS Permit Control Facility and implement the PTW system for Company Operations. Co-ordinate and manage work permits and certificates for Construction, Commissioning and Operations in SIMOPS controlled areas. Ensure that all permits have the necessary documentation (e.g., Isolation Certificates, Method Statements, drawings, etc.) and Task Risk Assessments have been performed for all relevant tasks as required. Participate in daily / weekly planning meetings, and PTW Monitoring and Assessment program.

Manage PTW issues at the Permit Control Facility (with Construction & Commissioning).

Coordinates and manages permits with Construction, Commissioning and Production Operations.

Works closely with Company Operations, Maintenance, Technical Services and QHSE staff to ensure safe and environmentally responsible operations at all times. Be a focal point for Construction and Commissioning Teams to ensure adherence to safe work systems (Permit to Works, SIMOPS, Task Risk Assessments, etc.)

Reports on PTW compliance, and participates in PTW Monitoring and Assessments on frequent basis.

Reviews Job Safety Analysis, Method Statements and Task Risk Assessments for worksites / permits.

Cooperates closely with Field HSE, Emergency Response and Security Teams.

Maintains PTW registers; and checks Long Term Isolation and Bypass Safety Device registers.

Performs other similar or related duties as assigned by the Field HSE Superintendent or Field Manager.

Complies with all Company Health, Safety & Environmental (HSE) policies and procedures.

Conducts all work activities in a safe and environmentally responsible manner by not taking on any task(s) that are perceived as unsafe and if in doubt, consults SIMOPS Coordinator before proceeding.

Participates in HSE meetings and company QHSE promotional programs.

Conducted safety audits for subcontractors to ensure that their safety program are implemented based on the permit to work system.
Initiated and conducted activities to promote safety and awareness of the workforce.

Analyzed all permit issues to determine the violation and field safety standards, and developed preventive measures to avoid accident recurring.

Supervisor’s / Engineer’s are aware that before they start their activity they must conduct safety talks and approved the job safety checklist and endorsement sheet.
In every safety site permit audit workers were informed they must identify safe and unsafe practices before their activities, to avoid any injuries and accidents. Increased permit audits to obtain the required level of acceptance and awareness of a workforce.
To provide effective database that can easily give the required items especially in a daily work activities. Tracking and traceability had been efficient so far with no comments from lead.
Permit activities are continuously monitored and comply to all requirements.
All engineers and supervisors were informed on the importance of their endorsement of each workforce eg. Requirement safety talks, job checklist before initial execution of work.
Provided signage’s for identification in each work area with FWP / EM and contact details.
A plot plan / work plan of PAC and PESEK were identified to the specific areas of what activities were conducted and posted in area 18 room 3 to establish plot plan on a weekly basis.
All permit applicants are aware that at the end of work permit durations; this needed to be closed by signing and returning the permits to IRF Tankage and Facilities Permits Coordinator.
Awareness of every IRF Tankage and Facilities Staff (Superintendent and Supervisor) for the preparation and information of what activities will be executed.

Nature of Duties

Duties and Responsibilities - Permit Coordinator:
In-charge of the issuance of the work permits.
Monitor the PTW from the application time up to the closing of the permit.
Inspecting and monitoring the PTW issued on site.

1.The applicant must be PTW qualified by CTJV & RasGas will apply for the relevant work permit from the permit coordinator.
2.Applicant will fill up the PTW forms and submit to the Permit Coordinator with the relevant drawing and method statement.
3.Permit Coordinator will check & fill additional safety precaution, if necessary, and will initial at the upper left hand corner of the PTW forms.
4.Excavation permit will be signed by the Civil, Mechanical, Electrical, Instrument, Safety and Construction Manager or his delegate.
5.PTW control number for excavations will be issued by the Permit Coordinator and will be logged / registered on the PTW log book. Other permit will be logged / registered also on the log book.
6.For the other permit: (Hot work, Cold work and Confined Space Entry)
1 to 3 will be followed
Gas testing will be done by qualified Gas Tester
Performing Authority and the Operating Authority will sign at the same time, after checking the work area.
Permit Coordinator will check/monitor daily the permit to work on site.

Review and analyzes the Method Statement, Job Hazard Analysis, Permit to Work System and Heavy / Critical Lift Plan submitted by subcontractors for approval.

Monitors the application of HSE Standards and Codes of Practices, provides advice and support in area of PTWs, isolations, chemical handling etc.

Work Performed as HSE Field:
1) Accidents/Incidents Investigations:
Investigates al accidents/incidents involving personnel, plants and equipment including near misses in area of responsibility,
Studies causes and reports to Line Supervisor recommending corrective preventive measures keeping area management informed,
Ensures implementation of approved ones,
Statistical reports of incidents/accidents, highlighting and analyzing problems areas an recommending preventive measures

2) Safety Rules & Procedures:
Develops/participates in developing local HSE Procedures as will be required by site,
Reviews all existing safety rules and procedures such as: emergency and non-routine work etc., in light of recurring incidents/accidents and recommends enhancements and amendments,
Controls implementation and monitors adherence to approved ones,
Ensures and educates personnel in the relevant rules and regulations

3) Safety Awareness/Educative Role:
Conducts safety courses to promote personnel awareness to safety rules, regulations and procedures, including: usage of fire-fighting and life-saving equipment,
Liaises with work associates and area management and conduct Specialized Safety and Environmental courses and participates in developing/enhancing checklists,
Ensures that all amendments/enhancements of safety rules, regulations and procedures are conveyed to all personnel through supervisors, bulletins or posters. Monitors adherence to and compliance with it.

Acts as Site Representative for the Environmental Management System
Reports environmental performance and trends
Participates in the preparation of and execution of emergency exercises
