I would like to apply for the above mentioned post vacant available in your company.

2) Currently, I am employed by Dayang Teras Shipyard Sdn Bhd as the HSE Exec and DPA ( under training). The company that I am working with now supplies Standby vessels for both Shell and Petronas. Besides carrying out the job as the Safety Officer , I am also being trained at the moment by the Technical Manager Cum DPA to be the DPA for future needs

3) Previously, I was working as the Administrative HR in the Admin. Department with an Oil & Gas Company which is a contractor with SSB for TMM – Top Side Major Maintenance Project – CAT3, Contract No. 590903. Before this, I was attached to the Operation Department as an Operation Technical Assistant (Onshore). I can converse fluently and write efficiently in English as well as in B. Malaysia. I am a fast learner who seeks for a rewarding job and a promising future.

4) I have taken a safety course (NEBOSH) in January, 2007 and have passed the test. For your information , I have taken the NIOSH – Safety And Health Officer Course which was completed in early August, 2007 of which I plan to take the exam this year end. I was also asked to sit in as an observer during the HSE safety briefing and Pre-mob briefing. I was also a statement recorder during one of the internal investigation and wrote a report on the outcome of the interview. If I am given the opportunity to work in HSE Department, I would not mind to travel nor would I mind to start from the bottom in order for me to learn more.

• as DPA/HSE Executive (15th January, 2008 till Present)

─ Prepare HSE Plan and ISM documents and doing documentation

─ Monitor and verify the safety and pollution prevention aspects of the
ship operation

─ To verify and monitor DTS Safety Management System as implemented
and maintained by DTS’s line management

─ To organised DTS’s periodical quality audits by his team

─ To coordinate external audits to maintain DTS’s ISM certification

─ To carry out shipboard and shore-based audits of the SMS

─ Investigating reported non-conformities

─ Ensuring that the SMS is understood by sea stuff and office employees

─ Identifying training requirements with respect to shore based staff
involved with the SMS

─ Ensuring that adequate resources and shore-based support are applied, as
required

─ Reporting directly to the Managing Director on significant issues within
the SMS

─ Ensure that all corrective actions are implemented

─ Endure that the implementation of Company’s Safety Management
System is consistent with the requirement of IMO (ISM Code), Flag
State, regional maritime and Classification Society, by tracking and
verifying in the world wide website (www) and shall verify the
Company SMS

─ To carry out Site Inspection Visit (SIV) and to accompany top
management for MIV

─ To do sea trial before vessel sign-on again and after maintenance

─ Others as per delegated
